| Effective Date | Date the NDA takes effect |
| Purpose | How Confidential Information may be used |
| Nda Term | Period for sharing Confidential Information (e.g. "1 year", "2 years") |
| Confidentiality Term Start | When the confidentiality term begins counting (e.g. "Effective Date" or "date of last disclosure") |
| Governing Law | State whose laws govern the agreement |
| Jurisdiction | Courts with jurisdiction over disputes |
| Changes To Standard Terms | Any modifications to the Common Paper Standard Terms |
| Recipient Signatory Type | Whether the Recipient signatory is an entity or individual |
| Recipient Signatory Name | Full legal name of the Recipient's signatory |
| Recipient Signatory Title | Title/role of the Recipient's signatory (entity only) |
| Recipient Signatory Company | Company name for the Recipient signatory (entity only) |
| Recipient Signatory Email | Notice email address for the Recipient |
